Julian’s Key My Health Passport is a communication tool designed to allow anyone to share their healthcare needs and conditions with their healthcare providers. The App is designed to assist people with disabilities who may struggle to articulate their needs but it can also be used by anyone seeking care from medical and healthcare professionals.
This free app is specifically designed for residents living within the West Moreton catchment.

• PIN protection to keep your personal information safe;
• Multiple Health Passports can be stored in the one App;
• Easy App editing and updating function;
• Ability to email completed Health Passport to medical providers.
This app was commissioned by West Moreton Hospital and Health Service with the support of Queensland Health.
